StrengthUnited’s Community Public Health Team (CPHT), Healthy Connections, Conexiones Saludables is a collaboration between StrengthUnited, Northeast Valley Health Corporation, and the Los Angeles Department of Public Health, serving the people of the Northeast San Fernando Valley area. Community health workers are going door-to-door to engage individuals in completing household health assessments and connecting them with local resources based on their needs.

In its first month of door-to-door outreach Healthy Connections, Conexiones Saludables visited over 600 households, introducing families to the team, the project, and helping connect community members to resources in the community. Each year, the Healthy Connections, Conexiones Saludables team will visit over 8,000 households. The team has successfully completed assessments with several families, and case managers are actively contacting these community members to link them to necessary services. Healthy Connections, Conexiones Saludables also hosts community gatherings for community members to have the chance to explore the program, discover local resources, and actively contribute to decision-making processes that benefit the community.

Impact: Each year the gap between the needs of the communities that we serve and the level of funding that we receive grows wider. Violence, hate crimes, and gender-based crimes are on the rise. StrengthUnited is stretched beyond its financial capacity.

As the need grows and our financial resources are stretched, there is also the unfortunate threat of a 30-40% reduction in funding (due to budget shortfalls) by one of our large funders. As a result, we would not be able to provide services to 750 clients.
